Leak at U Keg Coupler

Had my gas run out on a keg of Guinness. Ran the soap bubble test to determine source of the leak and found that the leak was at the connection between the coupler and the keg itself. Additionally, I have air pockets forming in my beer line. Any guess on what has failed? Thanks in advance!

There is an o-ring on the keg and an o-ring on the coupler. One of the two has a nick in it, or you haven't spun it all the way onto the keg. Those are my guesses.
