How long has that keg been connected after awhile beer can over carbonate.Turn your regulator off bleed air off keg and check to see if regulator is returning to the pressure you have it set at.temp should be 38 - 40 degrees any warmer your beer will make its own bubbles.Dirty lines will affect the taste and on certain runs make your beer flat.Once your keg is initialy (SP?)pressurized it will pour with c02 turned off.
