It could be the keg itself. If you untap the keg and look at the rubber seal that sits around the ball you see on the top there, you'll see a raised ring on the rubber ring. Dry it off and see if it looks chipped or mangled in anyway this often causes the symptoms you are experiencing. Also check your temp with a calibrated thermometer to be sure your temps are right. It could need servicing by a refrigeration company as these need recharging now and then.
